I hereby grant Accolade Community Theatre the right to take photographs and videos of my child(ren) (listed below) and of my family in connection with the production "Velveteen Rabbit.". I authorize Accolade Community Theatre, its assigns, and transferee to copyright, use, and publish the same in print and/or electronically.

I agree that Accolade Community Theatre may use such photographs and video of me with or without my name or my approval of the final product for any lawful purpose, including but not limited to such purposes as publicity, illustration, advertising, and web content.

I understand and agree that since my participation with Accolade Community Theatre is voluntary, My child(ren) will not receive any compensation. All photos are the property of Accolade Community Theatre and will not be returned to you.
